Adobe fill&sign on android an image icon missing under create signature

Seems like the app isn't loading correctly. Kindly close the app and reboot the device.

If that still doesn't bring the sign options, then 

• Check with a different file
• Make sure you are signed in to the app
• Nothing works? Uninstall and reinstall the app. [Note: uninstalling the app will clear the files under Recent view]
