After save, button gost

I created form and I saved. When I oppened the document for do modification and I've noticed that the radio buttons have disappeared from the screen. They appear partially in the "field" list, and I can no longer select them.

Do you know how to remove them?

(In the screenshot they should appear in front of each item: thermostria, Facade Solution, Parkopolis, Anticorrosion, Agroalimentaire, neutre, Oléo, Excelior, Aquasoft, Boiserie, déconature, Ikarlak)
