bad parameter

I am attempting to produce a filliable form.  

I get all the way to the end and when I go to send it I get this error;

i hit ok and the distribution continues.  

HOWEVER,  I never get the the email that I sent to myself as a trial run before I send the form to my boss.  

I have been spending way to much time on this... it would be easier and cheaper to have him use a pen and send me a picture.
