Button to make checkbox readonly and a strikethrough over the text

I'd like the buttons at the bottom of the page to make the relevant checkbox readonly and a line through the text.

(When a service is unavailable, the staff can make the relevant ones read only and then send the form to the customer.)

In a PDF form, it is much easier to gray out the texts than to strike them out.

I'm not that advanced yet to understand.

I've also changed the form. The radio buttn at the bottom if yes is selected then it disables the relevant checkbox at the top.

Is it possible to provide some direction?
