Cannot create accessible forms with radio buttons in Adobe Pro 2025

I am attaching a picture of my version release of Adobe Acrobat 2025.  I am also attaching a form that I need created that is accessible.  We have spent hours using the tools in this version of Acrobat trying to make this form screen readable.  Radio buttons ARE NOT ACCESSIBLE in this version of Adobe.    I'm not sure what happened with this latest release but it is almost impossible to create different groups of radio buttons in the same document. 

Radio buttons are absolutely necessary for this form as we do not want people checking more than one box in certain areas.  

 

 I have not found anything addressing the new behaviours of radio buttons in the 2025 software release and the knowledge base articles for previous versions  do not apply.  I would appreciate someone creating accessible radio buttons on this form and then attaching the result with instructions of the steps taken to make them accessible to this post so that I can learn.  

 

We are a government entity and have until April 2026 to make our forms 100% accessible.  We have hundreds of forms that require radio buttons.  Please advise.

I just looked into the attached file, and thanks for sharing it; otherwise, it would have been tough to understand the core issue. 

 

You have applied Radio buttons correctly. The only thing you need to do is change the "Button Style." Please check the screenshot attached. Your selected choice is "Square"; you need to change it to Circle. 

 

 

 

Also, you may want to install pending updates for Acrobat: https://adobe.ly/46wR0Lq

 

To check for any pending updates, launch Acrobat > Help > Check For Updates.

Thank you - I wasn't aware of that option.   One more question.  What do I do in situations where the tool taggs the field as a check box instead of a radio button?  I'm also having issues creating seperate groups of radio buttons on the same page.  Can you provide a link to a page that addresses these tasks with this specific version of Adobe 2025?

This is not the answer to your question, as @Tariq Ahmad has provided.  Just a comment to this:

"Radio buttons are absolutely necessary for this form as we do not want people checking more than one box in certain areas. "  You can make check boxes mutual exclusive (only one can be checked) by naming them the same and providing different export values.  I prefer this in most cases because once a radio button in a group has been checked that group can't be unchecked manually.
