Creating fillable forms...

I have a sheet of die cut labels, 18 up on a sheet & want to create fillable forms for each position.

I also want to have 3 radio nuttons to the right of the content that will do the following, first button will reset that form. Seconf button will 'turn on' all the content so it will print & the third button will 'turn off' all the content , so I can print them as I need them. 

Could I create one & then step & repeat them to fill up my sheet?

Any suggestions?

I agree with Try67 that radio buttons are not necessarily the best choice for resetting the form.  A regular button if fine for this operation. 

 

There are several print options in Acrobat. One of them prints only the form fields.  So printing just the fields does not require turnning off content. 

But if you want to do it by hiding and showing page content, then the page content must be attached to an OCG Layer.  

I'd use a check box this action. 

 

And Yes, you can create a single field and "Create Multple Copies" on the page at a specified separation. This is an option on the Right click menu when the cursor is over the field, and Acrobat is in Form Prep mode.  However, just this may not get you the exact positioning you want.

Yes, you can do that. But I would not use radio-buttons for those actions. Use regular buttons fields, instead.
