difficulty filling out form

I received this form from a colleague in Europe. I am not able to fill out the attached form. When I type information in, it turns into a series of small boxes. Does anyone have any suggestions? Thanks in advance for your help. Arvind

The application they used to create it (Nitro Pro) did not do a good job of embedding the font they selected (ArialMT) for the fields. I replaced it with Helvetica in Acrobat and now it works fine. See attached.

In the future they should use a different font, or a different application to create the file.
