Export data from fillable PDF to database?

We currently have fillable PDFs online for common forms. Here is one of them:

My boss wants to have them fill it out, then they hit a button labeled Save or Submit and the field data would go into our database table. Plus, the PDF would print.

I'm searching in Google, but I haven't found a solution yet. Is there one?

It's not possible to export directly from a PDF form to a database. You can export the data to a plain-text file, though, and then write a separate command to import it into the DC, independently of Acrobat.

Another option is to submit the data to a web-server, which in turn can take it and insert it into the DB (if the DB is also available on that server).

Using the Merge Data Files into Spreadsheet command, under the More button in Prepare Form mode.
