Fillable form: some fields do not show up

Hi! 

I'm trying to create a fillable form and I repeated the same process for all fields, but during preview some of those fields do not show up at all. I checked the settings and the fields are visible and filling forms is allowed. Attached is the screenshot from my form. 

Has anyone seen this problem before or knows how to solve it? Thank you!
