How do i create a custom dynamic stamp in adobe acrobat dc

i want to create a stamp

Entered in Dragon

by Mrs Thomas on current date & time

I then want to put it on my quick access toolbar, so I don't have to keep opening stamp.

All help would be really appreciated.

You may be better off just modifying one of the existing dynamic stamps in Acrobat vs. creating a new one. See here for instructions about how to do that:
