How to add header image in Adobe Pro DC

I understand how to add a text header and footer. My question is how to add an image header into all pages of a PDF document?

You could create a button (Rich Media tool) with an image in it > right-click > and Duplicate Across Pages. Note that if there is a change, you will need to delete the button from each page. It's not a header/footer.

The other answer is to do it in the source document and make a new PDF. That's not always a choice.
