How to collect consents to photography using digital signature

Hello everyone!

 

I need to collect consents to photography and picture usage from guests on my event. For this purpose I am going to create a document with general text and two adjustable fields: 

 

- Name & surname (text block)

- person's signature (I'd like the to draw a signature with their finger on their phones / tablets)

 

How guests will receive the document? I am going to create a document, save it to the Google Drive and link it via a QR code. Ina nutshel, guests will come to my event, scan a QR code, open the file, fill in their name and surname and sign the document in the created field using their finger or stilus.

 

I've tried different ways to edit the document: to create a form with specials fields (for text and for signature); to sign the document, but I have never received a document with two necessary fields that I can save and link to a code. 

What is the process?

You should use something like Acrobat Sign or DocuSign for this kind of thing. Doing it from a shared file on a cloud will not work, as each copy will overwrite the previous one. You need a distributed solution where you send one file, and have each response save as a new file on the server.
