Please explain what exactly you want to achieve, as the original post is not very clear...

I see what you mean, but this kind of dynamic form is not really possible in a PDF, unless:

- The entire form is composed out of fields, or

- The parts to show/hide are entire pages.

Either way, it will require quite a bit of scripting know-how, especially the first option.
