How to make a fillable employee application for online use

Hello

I am trying to convert our employee application to a fillable one to use on our website. I am not sure how to do this and could really use some help, very new to this program. Thank you

Is it a PDF file? Does it already have form fields?

Yes it is saved as a PDF file, I want to put it on our website so potential employees can fill it out and sign then submit it

OK... So what's the issue? Just put it on the website and provide the link to the employees.

If you want it to work correctly instruct them to download it and open using Adobe Reader, though, not inside the browser window.
