HOW TO PRINT CMS-15000 CLAIM FORM ON HP LASERJET M209dw

Hello, I'm in need of help with printing  "text" only for a preprinted CMS-1500 claim form from my computer with no billing software. When I print the fillable claim form, the "image, graphics" print as well on the red preprinted claim which voids the claim. I can only print the fillable text boxes. Please help. I know it is much easier with a billing or provider software, but they are expensive and I have been able to print my claims in the past with no issue. I recently updated adobe package and since have not been able to print the claims correctly. I have a HP LaserJet M209dw printer and using

In the printer dialog under Comments & Forms, select "Form fields only".
