Image placeholder in adobe pdf chrome extension

Is here a way to replace an image (it's a placeholder image we created using adobe acrobat, which we then upload online) when it's in a chrome window? It shows that the image placeholder is clickable, but when we go to click it nothing happens. We have also tried downloading the adobe acrobat chrome extension in the hopes it'll help, but no luck. 

I'm attaching a sample pdf below (please drop it on chrome to see the issue)

Thanks for sharing the form.

Have checked the form and put that on the web as well. The form works well on the desktop version of Acrobat and Reader.

However, to make it work on the web, I believe the same script will not help. We do not have that much expertise in writing JavaScript. 

Hence, looping in @try67, who can further look into this and see if this can be done.

Most browser PDF-plugins don't handle scripts correctly. The solution for that is to instruct the users to open the file in Reader. There's really not much else that can be done about it.
