Interactive Form Issue

I have an interactive PDF form that has various checkbox fields. I start off using my blank template, choose various checkboxes and then save the document. Then I have to create another document from the original blank template and save the 2nd PDF with a differnt file name, When I open the 2nd document, the checkboxes I chose on the 1st PDF are chosen. I cannot figure out why this is happening. If anyone has had this issue before and know the fix, I would greatly appreciate your help. If you need me to attach the document, I will gladly do so. I just need to make changes to hide propietary info since this is a work document.
