The rich media tools, in edit mode allow you to insert a video to include in the file.

If I understand well, you are looking for a form field that would allow users to attach-upload-insert a video in the form.

You can use the 'Prepare Form' set of tools to add an image field in your page.

When a user clicks on the image field, they will be asked to choose a file from their system, including mov, mp4.
