Have Adobe Acrobat XI Standard installed, but cannot get PDFmaker to show up as an Add-in to Microsoft word. Is PDFmaker a separate product or is it included in Acrobat Standard?

Adobe PDF maker is not a separate product, it is a part of your Acrobat Application.
The Add-in gets installed when you install your Acrobat Pro or Std application but if it is not visible in your Microsoft app, it may have been disabled.
You can try the following steps to enable it:
1. Open any Office 2016 application and click File > Options > Add-ins.
2. Select COM Add-ins next to Manage and click Go.
3. In the list of available add-ins, find the Acrobat PDFMaker COM Add-in and check if it's enabled. If it isn't, check the box to enable it.
