My access to Prepare Form seems to be gone

My access to Prepare Form appears to be (now) unavailabe. I've been working on a Form off and on for months - as recently as a month ago - but when I tried to work on this form today, the Prepare Form tool is now showing as "ADD" instead of "Open".

Thanks for reply. For a while yesterday Adobe was prompting me to upgrade in order to use Prepare Form. After exit and laptop restart the issue resolved.

When you select ADD, does Acrobat add the Prepare Form?

Can you post a screenshot? It's not clear what you're referring to by "ADD instead of OPEN"...
