My information has disapeared. How do I get it back?

Question

I filled out an online PDF form and tried to save it. The form saved but the information I entered is invisible except when I click on the pane where it should be except the check boxes I have checked. How do I get it back? This has to be on the originator's end because I saved another PDF and it is fine.

I am an individual using Reader only.

Where is that form from?

Did you contact the form supplier?

Can you share the form with us? (empty, no need to share confidential data!)
