For some reason I can no longer add a person name where it automatically adds to the 'name' field. When requesting signatures it used to come up with person's name and email. Now it's just email. Attached is what I used to have.

If the name field is not available, please check if it is enabled in the account settings. Also, check if you have selected any authentication methods, such as KBA, Government ID, Digital Identity Gateway, or Digital Signatures.
