Please help I cant type in all the fields

I'm not sure if I'm doing something wrong.

When I scroll my mouse over the areas that need to be filled out, it allows me to click as if I should begin typing - however, when I type, nothing happens. I attached the pdf for reference. If anyone has any tips on how I can get this to work, please let me know!

There are more than 200 fields in this file, but most of them are disabled or hidden. It's possible a script was supposed to make them visible (probably when you make a selection of some kind), but it contains an error that prevents that from happening. Report this issue to the authors of the file. There's nothing you can do about it on your end.

Do you experience this issue with this particular PDF form? Did you receive the form or created the form?
