Pointer has a box attached

That's the default look for the Selection Tool.  Switch to the Hand Tool, and the icon will be different... and it will shift if you roll over an image, and then text on a PDF page.  Nothing to worry about, really.  you can visit your Preferences and change some pointer/hand tool settings to see if one feels better to you.
