Remove checkmark Vers. 11.0.23

Came from WA Employment Security Dept., initially checked box, then realize should have been unchecked. Checked correct one, but now can't remove orignal check mark,

Any ideas or am I out of luck?

It's probably a single radio-button field. You'll have to clear the file to get rid of it, unless you have Acrobat in which case you can clear it on its own using a script.
