Requesting signature- wont upload

Hi 

I am look for some help with digital signatures, there seems to be a program error when I go to the next step of selecting where the signature goes the screen stays loading at 72% (see pic below)

Would really appreciate some support to rectify this problem, have tried calling adobe for support and continually get cut off.

We were experiencing server issues when you initially posted on the community. Would you mind letting us know if the issue is fixed now? 

If not, please share a screen recording of the entire event for a better understanding and assistance.
