You will need to download the PDF and view it in a Acrobat instead of letting the browser be the viewer. e.g. Some type of forms do not work properly in some newer browsers.

The file works fine. You have to make sure you download it directly, not open it in the browser and then try to save it as a PDF. I'm zipped it up to make sure that won't happen when you download it. You can get it here:

Same issue when I download the file you provided. I tried uninstall the program but issue still here.

For Mac users on macOS Sequoia, download the camera connectivity version via the "Download for MacOS Sequoia" link. If you're on Sonoma or earlier, use the "Download for Mac" button, and check your OS version by clicking the Apple icon and selecting "About This Mac."
