Text box displaying its name

I was wondering if there was a way to make a label viewable in a PDF text field form.  I am making my PDF a form with a fillable text box in it.  Well, I'd like the box name to show up when someone opens the PDF, not just if they only go into the edit form view. Some of the fields i have are date and time formatted. My issue is that I want them to display "date" or "time". 

 

Any suggestions

You can use the field's tooltip to do it. If it's formatted as Date or Time it can't hold the string "Date" or "Time" as its value...

Can this not me accomplished without being in Custom format????
