Toolbar Icons not showing up in

Anyone know why the toolbar icons show up in the customize section as the default selected ones (you can't remove them), but they won't show up in the actual toolbar as shown below:

This works for me. I am running the lastest version of Acrobat. I would see if there are any updates available (under Acrobat's Help menu), and if that is not the case, I would uninstall and re-install Acrobat using the Acrobat Cleaner (https://www.adobe.com/devnet-docs/acrobatetk/tools/Labs/cleaner.html).
