Unable to use Fill & Sign option

I have already created a signature and initials. When open up my next 2 documents the pen and Sign on the toolbar is inactive. But yet I have already created, signed and saved a signature for another document I signed. HELP!!!

I got into a loop a few days ago trying Fill & Sign. However I discovered that if you only want to sign with stylus, use "Comment" function. It works flawlessly. Just click the pencil tool to sign with mouse or stylus.

When I clicked Fill & Sign, then "Sign yourself" icon, the first popup shows Add Signature. Add Initials. When I clicked Add Signature, the entire screen is overlayed with dark grey, with no popup or window to continue. Not even a "Cancel" option. The 3 dots on the top left corner do not work. In effect the window hangs and I have to force quit. I did this several times. It's maddening.
