Use QR code to direct people to an electronic version of a form that they could then fill out.

I would like to be able to put a QR code on a form that when scanned opens an electronic version of that form that could be filled out. I'm curious as to whether this is even possible on a personal account.  If it is possible, I would like the filled form to be emailed to me. Right now, I am interested in possibilities or direction in how to go about finding a solution.

 

I am attaching some examples of the forms I am doing this for.  I haven't begun trying to make them fillable yet.  This is just an example of what the paper form would look like and what kind of information might be needed.

A bit of info about a QR code. A QR code is just a way of putting some text in a barcode. When it's scanned, the text comes back. It's always text, it can't be a picture or a file. But what's usually done with a QR code is that it has a web link (http:...) So when you scan the code, the scanner might (or might not) go off to a web page. So, you'd put the form on the web page.


But that's where the problems start. Email is a TERRIBLY unreliably way to receive a form. Not really suitable for any important information. And you'll get a log of tech support calls about the form not working in such and such a device. Best to link to a Google Form or something like it. It won't look pretty but it will do the job.

Could the link be to a PDF on a SharePoint or drop box?  I want them to use reader, because at some point I hope to get to forms that require signatures. So, what would be the best way to notice that a new form has been filled in?

Sure it is, just add the QR code as an image to the file.

But why have two versions of the file in the first place? Why not just distribute a fillable version, and if someone wants to fill it in by hand they could simply print it and do so?

The locations where the form is do not have printers.  I also work with some people who refuse to embrace technology, including a State Oversight that still insists on paper copies. So, regardless of my desires, it must be printed at some point. I am trying to address some concerns of employees who don't like coming to the office.  I am also trying to convince my boss that this might be a solution. (or something similar) I am trying to start simple.

So how will the people without access to a printer supposed to fill in the non-fillable version of the form? Do they use the commenting tools in Reader or something?
