Why am I not seeing the format tab?

You probably have adobe sign form. Select 'prepare form' tool, click on 'More' then click on 'Revert to acrobat form'.

And in the future, do not tick the box that says "This document requires signatures" when creating the form.

Try this: Prepare a Form > More > Remove e-signing options.  It worked for me!
