Auto extract pages with headings

Please allow me to mumble my way through my question..

Is it possible to extract a PDF Book to single pages with the appropriate Title? If a book consists of 7 pages with titles such as:

SongBook

Index

Silent Night

Deck The Halls page one

Deck The Halls page two

Deck The Halls page three

Backpage

Can Adobe extract the above songbook to single pages with the titles?
