Well, the Reader Extension works like a digital signature: If just one bit of the PDF has changed, it becomes invalid. This can happen, when the form was de/compressed by a 3rd party application (e-mail, file transfer etc.) or when it has been corrupted during copying or saving. You'll have to (find someone) to apply the Reader Extension to your form again, i'm afraid.
