Cannot save the completed fillable pdf form and background data lost

Hi, after I used Acrobat Reader to fill in the data in fillable pdf form and save, the background wordings lost and cannot save as shown in attached screen cap, it happened very frequently, please advice how to fix it. Thanks in advance.

Very hard to say what the issue is without seeing the actual file. Can you share it?
