Text boxes get linked to multiple pages

Can someone help me understand what I should do when the same text boxes appear in multiple pages and causing the duplicate text boxes to appear in inappropriate places on the other pages of the document? When I delete those text boxes, all the other duplicates, including the original, disappears. I asked before, and the answer was "if the fields have the same name/label, this will happen. Please make sure that the fields on different pages have different names/labels." I don't exactly know how to do that. I want to put a stop to that somehow, but couldn't figure it out. Thank you for your assistance.

Report this issue to the author of the form. They need to fix it on their end. There's nothing you can do about it using the free Reader.

You need to rename the fields. The free Acrobat Reader does not let you do this. You will need Acrobat Pro.

Prepare Form tool > Double-click field to open Properties > General tab > Type new name for field
