Is this for

* A server?

* A workstation for someone who pays for Acrobat Pro subscription?

* Somehow to work without buying anything from Adobe?

* Something else?

So, are you prepared to license Acrobat Pro for each of your users? If you are, we can help.

To add fields you need an app which can do this AND which has a programming interface. Acrobat Pro is such an app. The programming interface is described in the Acrobat SDK, which is mainly documentation you would need. (By text boxes do you mean form fields?)
