Adding a signature, date and comment to a pdf

I am trying to find the best way to send employees a pdf that they can sign off, like approving invoices.  I need their signature on it, the date and for them to be able to add comments. I have looked at Adobe Sign, but that doesn't really work as I just want to be able to forward an e-mail to them and then they sign it etc. and then send it back.  Has anyone got any ideas?

Use a signature field and a text field. The former automatically includes the exact date and time it was signed.

The latter can be used to enter comments, either before or after signing. You will need Acrobat to do all of that, though.
