If I create a form with MS Word that includes check boxes, radial buttons, and dropdown boxes, will those controls be imported into Acrobat or do I have to create them again from scratch when I create an Acrobat PDF form from the Word document?

You have to create the again from scratch after you convert the file to a PDF.

Hi Heather,

If you have InDesign, you can create pretty much all you want and need  there and then save it as a PDF.

Here's a brief video tutorial on how to do this and it shows off a whole bunch of the potential options.

Form fields created in InDesign do not always work in Acrobat or require some fixes. OpenOffice.org the form fields and options carry over to the PDF.

It cannot be done in MS Word either using MS's Save As PDF/XPS or Adobe's  PDF Maker. It is possible with other Office Suite programs like Apache Open Office.
