Prefilled fields are

When I drag and drop a document into Adobe Sign and then go to preview the file and add the signature fields, all the prefilled fields of my document are blank.

Any ideas how to fix this?

@realistic_knowledge2130 I believe it's because Adobe Sign does not automatically recognize and populate data from form fields created in other programs. Adobe Sign treats the uploaded document as a flat file, like a PDF or an image, and doesn't read the underlying form field data.

This is a change then, because I have always uploaded Adobe Acrobat Pro forms with pre-filled fields and Adobe SIgn has always recognized and autopopulated the pre-filled forms.

Please let us know if you are using the feature in the Acrobat desktop application or the Acrobat Sign web. Could you please let us know how the form was filled out? Are you using the PDF form or any other format file?

More information would be helpful to further investigate this behavior.

I'm using Acrobat Sign web. The form is a PDF form filled out in Acrobat Pro. I've attached the form. I usually fill out the "Patient Information" and "Send my Information to" sections before uploading into Adobe Sign.
