Two copies of email are being sent each time

Every time we send out a signature request the customer is receiving two duplicate emails for the same request.  We also get sent two copies.   How can I stop this happening - Any ideas?

Here are my settings :

I have 3 Internal emails listed for the extra copies (we all received duplicate emails too!)

Hello NewlineASP-TQ,

I would recommend to open the Support ticket and provide Document details ( Sender's email, signer's email, document name and date) so that they can further check at their end.
