"Report a bug" Form Broken on Chrome

When I try to report a bug while in Chrome, the dropdown for "Topic" doesn't work. It drops down, but when you select the topic, it goes blank and is never able to select anything, yet it's required to submit a bug. Also, it clears all the fields above it each time. Thankfully it is working in Safari.
