Scheduled posts are not showing up in schedule but they're appearing on platforms

Randomly & without indication why, some of my scheduled posts are not showing up in schedule but they're appearing on platforms - sometimes more than once. It's embarrassing for my networks to see duplicate posts especially when there's no warning when I'm scheduling them.
