Make a model release

Hello!
I'm taking an online class in Professional Practices in Photography, and we're creating model releases.
I could bang this out easily in Illy, but I'm super curious about creating it in Firefly and I think that would be very interesting to the class, but I can't figure out how to tell Express to create it? How would this work?

I had very good luck creating pictures of what I thought my friend's backyard swimming pool should look like but I dn't know how to create documents, it has to have these elements:

  • Your Name or Business Name
  • Heading of Document: “Model Release”
  • Shoot Date and Brief Description:
  • All Parameters discussed: i.e “for valuable consideration”
  • Photographer Signature Line:
  • Photographer Printed Name:
  • Signature Date:
  • Subject/Model Signature Line:
  • Subject/Model Printed Name
  • Subject/Model Birthdate:
  • Subject/Model Address:
  • Subject/Model E-mail: 
  • Witness Signature:
  • Witness Printed Name:
  • Witness Address or Email:
  • At least 2 Photographer Contact info: Phone/email/web address/address
    Thank you in advance, a couple of years ago I basically learned Character Animator from the forum, and made a couple of episodes of a cartoon on youtube 😉

I have a school assignment to create a model release, and I'm curious to see if it can be done with AI in Firefly. Does anyone know how this could work? I haven't been able to figure out how to do it...
These are the elements it needs to contain:

Hi Sugar,

 

Firefly is not the right application to create a form, as it is not for document creation. Use Adobe InDesign (preferred) or Adobe Illustrator instead.

I was asking how to use AI. I can already do it in InDesign or Illy, that's not what I was asking for. 
So it's not possible to create a release form using AI?
