Scanned documents looked at date

Yesterday several of my scanned documents had the word Today on them as the last looked at date. They were that way when I opened up the Adobe Scan app. It seems as if my account was hacked. 1. Is that possible? 2. Can it be traced back to the specific person that may have hacked it?  3. What steps should I take now to prevent this.from happening again?

Sorry for the delay in response.

 

As you have mentioned the scanned documents had the word Today, could you share the screenshot of the screen where the word shows?

The scans generally shows the date in which the scan is taken not the day when it is open.

 

Try once to sign out of the application and reboot the device. You may also uninstall and re-install the application.

Check if the issue still occurs.
