Digital "blips" in playback?

This is a whole new thing - I've been using AA for many years with this exact setup, and suddenly this issue.  The only difference is that the speaker isn't me - I'm engineering.   And - I don't hear this while monitoring live!!

Please review the :07 sample attached.

Can you supply an example as a wav file? MP3s are notoriously crap when it comes to diagnosing anything...

Okay - if you didn't get this on monitoring, it's presumably because you monitored the input feed - either before it got digitised, or saved. Q1 -Was this a Waveform or Multitrack recording? Before you answer that, have a look at this:

The bit I've put a ring around is seriously distorted. The two arrows point to bits of 'silence' that have been recorded. The first one is interesting, because it's a small section where the encoding has effectively stopped mid-stream, and the second larger flat line is another example of the same thing. Q2 is 'what interface are you using?'

Did you have any more insight?   I've got another session on Monday and would like to avoid this going forward.   

Also, I researched & found "Input Monitoring".   I don't think I can do this as the talent will hear that feed while recording and it will distract him.   

Regardless of using IM or not, I'd really like to solve this - is there any more info I can offer that will help to that end?   I'm at a loss.
