How do I put the address bar back at the top of the window?

When I go to Window and check and uncheck PathBar, the highlighted element loads / unloads:

Where do I grab to but the folder's address at the top of the window?

I was able to recreate your problem (it took me a while). The only way I was able to get the PathBar back to it's normal position was to use Reset to Saved Layout

I went to a workspace that has the path showing, and modified that workspace and then saved it.

Yes, but the path bar had been dragged into another panel and couldn't be dragged back out. There was no way to grab it.
