Has anyone encountered a dark grey bar that appears over the answer choices upon retaking a quiz?

This happens only on multiple choice questions, with shuffled answers, and upon retaking the quiz. The dark grey bar appears when you hover your mouse over the answer choice selections and actually blocks the text from view. However, when answering the multiple choice question the first time (prior to a retake), the grey bar that appears is much lighter and does not cover the text. No other question types had this issue, and it does not happen unless you select Shuffle Answers. That is the only common denominator.

I've never seen this issue.  Can you post a screenshot?

Thank you for the suggestion, Rod. We tested this by changing the theme, and the issue did reappear.

Just to clarify, we created our project template using the "white" default theme in CP9 and modified it to match our standards. Do you have any suggestions for ways I can try to fix the issue without having to recreate the entire template file? Although this issue is not a deal-breaker for courses, we would like to know why it is happening and if we can improve it.

Finally, I'm curious about the fact that this issue only occurs following three exact circumstances: 1) Only on Multiple Choice question types; 2) Only when Shuffled Answers is selected; 3) Only upon retaking the quiz (issue does not exist on first attempt). It seems the scenario is so specific...why only in these circumstances do we see it? Appreciate your opinion on that. Thanks again for your help!

Did you say this was a Template?  Are you talking about a CPTL file or a CPTX?
