I only want the user to have one click. they can then undo if they want to change their mind. But even when the No. of Attempts is 1 the user is allowed to click as many times as they want on the screen? Has anyone had this problem or know a solution?

This is normal behaviour for a hot spot question slide.  The ONE ATTEMPT relates to the number of times the learner can Submit their answer to the question slide, NOT the number of times they click on the screen.  You cannot prevent them from clicking more than once with this type of question slide.

The alternative is to create a custom hotspot question slide, but that means more work. Have a look at this thread:

Anyone know how to customize the "click" in Hotspot quiz? (Captivate 9)
