Weird behavior in Captivate 9 on our training server

1.  We have CP9 projects that all of a sudden in the middle of a quiz will have the narration from the last slide play. 

2.  In a different CP9 project when you click various smart shapes used as buttons the end of course survey will open.  This survey is linked from the last slide by pressing a button and the link isn't anywhere else in the project. This action wasn't consistent with the same buttons.  If you went back to that slide and pressed the same smart shape it would behave correctly and another smart shape button would pop up the end of course survey later on.

Find the audio clip in the Library and check the Usage for it.  If it is only citing a single usage, then its possible there is some action (e,g, On Enter Slide or On Success / On Last Attempt) for one of the question slides that has become linked to the audio clip and triggers a Play Audio action.  If that is NOT the case, then the only other reason is usually that your project has become corrupted and you may need to strip out the corruption by copy / pasting all slides into a new project shell.
